MEMBERS (5th grade age and older)

To register your child(ren), please download and complete the below forms. Completed forms must be emailed to yucaipaeaglespfc@gmail.com by August 10, 2025.

4. Uniform Agreement
5. Parent Volunteer (non-staff member) sign up form

VOLUNTEERS (18+ years old)

To volunteer, please download and complete the below forms. Completed forms must be emailed to yucaipaeaglespfc@gmail.com.

1. Staff registration form
2. Staff information
3. Volunteer vehicle form (21 years and up)
4. Adult consent form (sign, but do not date the form) 
5. Uniform Agreement 
6. If not completed last year, complete both

   2. Live Scan using the SECC form only
